Submission of this completed form to CSLB by a contractor meets that notification requirement.
Please be aware that when an HIS is no longer employed by a contractor, the contractor must notify CSLB within 90 days after the date on which the HIS ceases to be employed, using the Contractor Notification of Home Improvement Salesperson Employment Cessation form that is available on CSLB's website. Failure of a contractor to make the required HIS notifications to CSLB is grounds for disciplinary action.
